Doctors

Accounting for doctors

Doctors are among the highest earners in the UK and, as a result, among the most likely to pay more tax than they need to if their finances are not structured properly. Whether you are an NHS GP, a hospital consultant, a doctor with private practice income, or someone working across several different roles at once, the tax and financial rules that apply to you are more complex than most accountants deal with day to day. We specialise in helping doctors take control of their finances, pay the right amount of tax, and stop losing money to avoidable errors.

What we handle for you

We cover everything from day-to-day compliance through to longer term financial planning.

Self-assessment tax return

We prepare and file your annual tax return, declaring all sources of income correctly and making sure every allowable expense is claimed so your bill is as low as it legitimately can be.

NHS pension annual allowance

We calculate your pension input amount each year, assess whether you face a charge, and advise on your options including Scheme Pays if needed.

Limited company set up and accounts

If you earn private income and are considering using a limited company, we advise on whether this makes sense for your situation, set it up correctly and prepare your company accounts and tax return each year.

Expenses and allowances

From GMC registration and medical defence fees to CPD costs, specialist equipment and home office expenses, we make sure you are claiming everything you are entitled to.

Income tracking and reporting

We keep a clear record of all your income throughout the year, so your tax return is straightforward and there are no last-minute panics pulling everything together.

HMRC compliance

We manage all your deadlines and submissions, so you are never at risk of a fine, a late filing penalty or an investigation from HMRC due to an avoidable error.

What you can expect from working with us

We work with doctors throughout the year, not just when a deadline is looming.

An accountant who already understands how doctors are paid and taxed

Plain English advice whenever you need it without being billed for every question

A tax bill that accurately reflects your position with no overpaying and no nasty surprises

Your NHS pension position reviewed so you are never surprised by a large tax charge

The NHS pension annual allowance is one of the biggest financial issues facing doctors right now

Many doctors are receiving unexpected tax bills of tens of thousands of pounds because their NHS pension has grown faster than the annual allowance that HMRC permits. This is not a penalty for doing anything wrong, it is simply a consequence of how the NHS pension scheme works and how HMRC calculates pension growth. But if it is not managed proactively it can come as a serious shock.

We help doctors understand their pension position, calculate whether they are likely to face an annual allowance charge, and explore options such as Scheme Pays, which allows you to have the charge paid directly from your pension rather than out of your own pocket. This is not something every accountant is familiar with, but it is something we deal with regularly.

Something we see often: doctors who have received a large annual allowance tax charge through their self-assessment but were never told it was coming and had no cash set aside to pay it. With the right planning this is avoidable, or at least manageable, if you know about it early enough.

If you have not had your pension position reviewed recently, it is worth doing before your next self-assessment deadline.

We help doctors at every stage and in every setting

Your financial needs as a doctor depend a lot on your career stage and how your income is structured. We support all the following.

01

GP partners and practice owners

GP partners share in the profits and losses of their practice and have their own personal tax obligations on top. We handle your partnership accounts, your personal tax return, and advise on how to structure your income and drawings in the most efficient way.

02

Salaried GPs

Even as a salaried GP you may have additional income from out of hours work, extended access sessions or private reports and letters. We make sure all of this is declared correctly and that you are claiming every allowable expense against it.

03

Hospital consultants

Consultants often have NHS employment income alongside private practice income, sometimes earned through a limited company and sometimes directly. We help you structure this correctly, handle your self-assessment, and manage the tax implications of private fees.

04

Doctors with private income

Whether you see private patients, write medico legal reports, or do private work alongside your NHS role, any income outside of your PAYE salary needs to be declared and taxed correctly. We make sure this is handled properly and that you are not overpaying.
